Personal care, pressure-sore prevention, monitoring: a daily visit, following a care plan drawn up after a home assessment.
Dressings, removal of stitches, monitoring of vital signs: the rest of your care, at home.

Yes, that is the “Chronic Care” plan: four visits a month (blood pressure, blood sugar, taking his treatment) and a follow-up sheet for his doctor.
Do we have to go to the practice?
No: the caregiver comes to you. The practice, in Yopougon, is open Monday to Friday from 8 am to 6 pm if you would rather come in.
